Goal Setting Worksheets

My boss just set a high sales target on me this month. I think i need a goal setting timeline chart.?
Where I can find such a chart? I guess books is one option but it's rather heavy on the theory and other stuff. I probably need a clean defined timeline on a worksheet to work on to reach my target.

If you're more concerned about a timeline chart than you are about selling the product (or doing the duties) then something is wrong. Pull out your lined tablet and make yourself a chart in 5 minutes and get on with your selling or your project.
